Travel to the Netherlands during Covid-19
All passengers arriving in the Netherlands from abroad are required to fill out a Public Health Declaration, also known as the Passenger Locator Form (PLF).
This form must be completed before your arrival in Holland and submitted to the Dutch authorities.
The Health Declaration Netherland is used to help trace any potential contacts of people who have been infected with COVID-19. It is also used to provide information to the Dutch authorities about where you have been and where you will be staying in the Netherlands.
The Health Declaration Form must be completed in English, Dutch or French. You can complete the form online or download it and fill it out manually.
Once you have filled out the form, you will need to print it out and present it when you arrive in the Netherlands.
The PLF Netherlands is valid for 48 hours. This means that you will need to fill out a new form if you are planning to stay in the Netherlands for longer than 48 hours.
Required documents for PLF Netherlands
When travelling to the Netherlands, you will need to present a Health Declaration form. This form must be completed before your arrival in Holland and can be obtained online.
The Health Declaration form asks for personal information such as:
- your name
- address
- contact details
- passport number
- date of birth
You will also be asked about your health history and whether you have any medical conditions that may require special attention.
You will need to declare any medications that you are taking, as well as any vaccination records that you have. It is important to note that the Netherlands requires all travellers to have a valid yellow fever vaccination if they are arriving from a country where the disease is present.
If you are travelling with children, you will need to provide their personal information and vaccination records as well. You will also need to declare any food allergies that your children have.
Once you have completed the Health Declaration form, you will need to submit it to the Dutch immigration authorities upon arrival. You can do this at the airport or at a land border crossing.
The Health Declaration form is an important part of the Netherlands’ public health measures to protect its citizens from disease.
It is important to note that the PLF may refuse entry to travelers who do not have a valid health declaration form. If you are found to be in possession of a fraudulent or incomplete form, you may also be subject to penalties.
How to apply for PLF Netherlands
This Passenger Locator Form is required for all travellers entering the country, and it must be completed prior to arrival.
There are two ways to apply for a PLF:
- Apply online through this website.
- Apply in person at a Dutch embassy or consulate.
If you are applying online, you will need to provide your full name, date of birth, passport number, and contact information.
You will also be required to answer a series of questions about your health and travel history.
Once you have submitted the form, you will receive a confirmation email with your PLF number.
This number must be presented to the authorities upon arrival in the Netherlands.
If you are applying in person, you will need to bring your passport and a completed application form.
The health form can be obtained from a Dutch embassy or consulate, or downloaded from the website of the Ministry of Foreign Affairs.
Once your application has been processed, you will be given a PLF number.
When entering the Netherlands, you will be required to present your PLF number to the authorities. You may also be asked to undergo a health screening upon arrival.
Health declaration and COVID-19
If you are thinking of travelling to the Netherlands, it is important to be aware of the requirements for entry. All travellers must complete a health declaration form and present it upon arrival.

If you have any symptoms of COVID-19, or have been in contact with someone who has the virus, you will not be allowed to enter the Netherlands.

Quarantine conditions
The PLF form must be filled out online before you travel, and you will need to show it to authorities upon arrival in the Netherlands. If you do not have a PLF form, you may be denied entry into the Netherlands.
If you are coming from a high-risk area for COVID-19, you will also need to show proof of a negative PCR test taken within 72 hours of your arrival in the Netherlands. You may be asked to quarantine for 10 days upon arrival, even if you have a negative PCR test.
Important information
The information required on the form includes your personal details, contact information, travel history, and health information. You will also need to provide your passport number and date of birth.
If you have any symptoms of illness, it is important to disclose this information on the form. You may also be required to provide a recent travel history if you have visited any countries where there have been outbreaks of disease.
